1. Click on decision tool located at the top menu of designer.

2. Click and drag the decision tool to the location you want to add it.

3. Click dropdown sidebar

 

4. Choose the multiple option

5. Type the question on the text field

6. Add more itens to your question if necessary

 

7. Set your nonconformity and write items

 

8. If you want to ignore the nonconformity in this matter select the check box

9. Select which connector you want to connect to that decision.
