1. Choose the activity tool.

2. Click and drag it to the flow.

3. Click once in the activity.

4. On the right side of the designer, click in.

5. Choose the “form” option.

6. Choose in the dropdown the title of the form that you want to associate with the flow.

7. Click in apply.
