1. Access on the side menu the proccess section.

2. Create a new form document or choose a form document already drawn.

3. Add a Decision tool.

4. Choose the type of decision.

5. Type your question.

6. Click in

7. Type the options you want

8. Add a value for each option

9. If you want to add another option, click in

10. Click apply.

11. After completing the editions of your form, click in publish.
